In order for the quest to become available, you need to have completed Main Quest 4, titled What Lies Dreaming. The quest is given by Belle, and you can start it in the Crooked Teeth area within The Whispering Glacier zone on Kairos. Furthermore, the DLC itself is required to begin this quest, which means it remains exclusive to Mad Ellie and the Vault of the Damned without any exceptions.
Once you finish talking to Belle, who is the leader standing in front of you at the beginning of the area, simply follow the quest marker in the northern direction to find the tomb within the Crooked Teeth region. Outside the cave entrance, a group of Scav enemies will be guarding the way, so make sure to eliminate them before you proceed further inside. The downward path is blocked by wooden planks. You can break them by jumping up and using a ground slam, which will cause you to drop into the cavern below. After that, swim through the freezing water and then climb out to emerge in the chamber located just beyond. You will find the ancient egg placed behind a force field that emits a red and orange glow. Grabbing it directly is not possible.
Here is what you should do:
Make your way to the highest point in the cave by using the various grapple points scattered around the room.
Once you are up there, look downward to spot both the ancient egg and the summoning circle beneath you.
From the ledge, execute a double jump followed by a ground slam that lands straight in the middle of the circle.
When you successfully perform a ground slam into the center of the circle, the force field will break apart immediately. After that happens, you are free to pick up the ancient egg without any further obstacles.
From the tomb, travel south while keeping an eye on the quest marker to guide you to the heart location. This area remains within Crooked Teeth, and more enemies are guarding it, so you need to eliminate every single one of them before you attempt to interact with anything. After the area is fully cleared, go ahead and interact with the heart, then activate the device that sits close by. Doing so will trigger the symbol puzzle, which represents the most challenging and tricky part of this quest.
You will see a master symbol positioned above the heart. Around the perimeter of the room, there are multiple matching symbols, and each one comes with a lever right next to it. Each perimeter symbol must be set to match the orientation of the master symbol located above the heart. Pulling a lever causes the symbol beside it to rotate or flip. Your objective is to cycle through orientations until every perimeter symbol faces the same direction as the master symbol. There are three formation stages to complete, and they must be done in the following sequence:
| Formation | What to do |
|---|---|
| First | Match all perimeter symbols to the first master symbol orientation |
| Second | Match all perimeter symbols to the second master symbol orientation |
| Third | Match all perimeter symbols to the third master symbol orientation |
Once all three formations have been completed in the correct sequence, the force field around the heart will dissipate and disappear. With the force field no longer active, you can proceed to pick up the heart without any further hindrance.
Return to the starting ritual site with the ancient egg and the heart. Place both items when prompted, then step outside the circle. The summoners call forth a demon named Greg. Suddenly, Mancubus interrupts, claiming Greg owes him a debt, and asks you to bring Greg to the Speakeasy. The summoners turn hostile, believing you are evil for knowing Greg's name. Kill them all, then speak to Greg. He goes to the Speakeasy alone, and the quest ends.

Completing the quest earns the following:
XP
Money
Eridium
SMG (Green or Blue rarity, per PowerPyx; notes Rare/Uncommon)
Cosmetic: Vault Hunter Head
The standout reward is the Vault Hunter Head cosmetic. The SMG's rarity may differ, so do not be surprised if you get one at a higher or lower tier than anticipated. In conclusion, the Summoner’s Gift quest in Borderlands 4 brings both excitement and rewards.
